EXCEEDING SPACE PROVIDED ON ESSAY PORTION WILL DISQUALIFY YOU.

FORMS WILL BE PICKED UP AFTER SCHOOL BY THE BOOSTERS, NO LATE FORMS WILL BE ACCEPTED.

TYPING OR NEATLY PRINTING ON PAPER FORMS (AVAILABLE FROM MR. SCHUSTER) ALSO ACCEPTED. (AGAIN, DO NOT EXCEED AVAILABLE SPACE.)
